**First Day Setup: The first day for your pup can be scheduled for either 7:00 AM or 10:00 AM, Monday through Thursday.

**Please Note: For your convenience, we will collect your credit card information in person, ensuring an easy and seamless process for automatic future bookings.

Waiver Consent & Agreement for Daycare & Boarding

1. I am the legal owner or agent for the owner of the dog enrolled in Pipsqueak Pups Daycare Inc.

2. I am responsible for any harm caused by my dog(s) while at daycare. I waive and release Pipsqueak Pups Daycare Inc. from any liability arising from the dog’s attendance.

3. My dog is in good health and has not shown aggressive behavior.

4. Minor cuts and scratches can occur even though dogs are supervised. My dog has not been ill with any contagious diseases in the last 30 days.

5. If unreachable in an emergency, I authorize Pipsqueak Pups to seek veterinary care for my dog at my expense.

6. I release and hold harmless Pipsqueak Pups from any liability or claims related to my dog.

7. Pipsqueak Pups reserves the right to remove a dog from daycare if it is not the right environment.

8. I will pay for any veterinary or medical expenses if my dog causes injury.

9. No refunds on services or packages. Packages expire one year from purchase and are non-transferable. There is a fee for late pickups.

10. My dog’s vaccinations are up to date. Vaccines are not guaranteed to prevent all illnesses.

11. Pipsqueak Pups is not liable for any problems that might develop with my dog if reasonable care is followed.

12. Minor cuts and scratches can occur. I authorize Pipsqueak Pups to secure veterinary care in emergencies at my expense.

13. I indemnify and hold harmless Pipsqueak Pups from any action if my dog causes injury or death to another dog or person. I will pay for veterinary and medical expenses if my dog causes injury.

14. My dog may be videotaped, photographed, and recorded for marketing purposes.

15. Pipsqueak Pups may remove a dog from daycare based on temperament, energy level, or sociability.

16. Pipsqueak Pups is not liable for any harm from grooming services. I hold responsibility for veterinary expenses arising from grooming.

Daycare Rules & Regulations

* We joyfully welcome dogs of all ages! Puppies must be on a vaccination program. Puppies or shelter dogs must have been in the home for at least two weeks before attending daycare.

* To ensure a safe and happy environment, all dogs over 6 months old must be spayed or neutered.

* For the well-being of all, up-to-date vaccinations are required. Proof of vaccination or titer testing is needed.

* Only healthy pups are allowed to join us. By signing this application, owners certify that their dogs are in good health.

* To keep playtime safe and fun, all dogs must be non-aggressive and not possessive of food or toys. By signing this application, owners certify that their dog has no history of harming or threatening any animal or person.

* Pick-ups must be completed by 6:00 PM. A late fee of $1 per minute applies after 6:00 PM, with no exceptions.

* Daycare passes have no cash value, are non-refundable, and expire 1 year from the date of purchase. No exceptions.
